Informed Consent for Participant of Investigative Project

Title of Project: Multi-view Management of Multivariate data in CORNET3D Principal Investigator: Dr. Nicholas Fearing Polys

I. THE PURPOSE OF THIS RESEARCH/PROJECT
You are invited to participate in a study to determine the best multi-view management of multivariate data from cognitive radio nodes' spectrum as a 3-dimensional visualization. In each trial, you will be presented with a pair of images/videos that represent a 3D visualization of CORNET3D which is an educational software that shows the activity of cognitive radio nodes operated by VT wireless research group. The results of the study will be used in a dissertation and published.
II. PROCEDURES
This study will be administered as a web survey using Virginia.tech.qualtrics.com; the navigation will be through a web browser, as a desktop application on your own time(not in the lab). You will be asked questions to judge the images/videos and also to rate the difficulty of answering these questions. We are not evaluating you or your performance in any way; you are helping us to evaluate different techniques of multi-view management. All information that you help us get will remain anonymous. You may be asked questions during and after the evaluation, in order to clarify our understanding of your evaluation. You will also be asked to fill out questionnaires related to your background with such applications and to collect your feedback.
The session will last approximately 30 minutes. The task is not very tiring, but you are welcome to take rest breaks as needed. You may also terminate your participation at any time, for any reason.
You will be given full instructions before the task. It is important that you understand the instructions before beginning the task. III. RISKS
The proposed experiment is a straightforward test of perception using a desktop computer. The physical components of this task are not stressful and there is
no physical strain besides that results from the normal daily activities. If you feel tired you can have a rest break at any time through the experiment. If you decide you do not want to continue, you can discontinue the study at any time.
IV. BENEFITS OF THIS PROJECT
Your participation in this project will provide information that may be used to improve the design of 3D visualization software. You may receive an abstract summarizing this research when completed, please send an email to the co-investigator (maaayat@vt.edu) asking for the results’summary and a copy of the summary will be sent to you.
You are requested to refrain from discussing the research study with other people who might be in the candidate pool from which other participants might be drawn.
V. EXTENT OF ANONYMITY AND CONFIDENTIALITY
The results of this study will be kept strictly confidential. Your written consent is required for the researchers to release any data identified with you as an individual to anyone other than personnel working on the project. The information you provide will not have your name, only a subject number will identify you during analyses and any written reports of the research.
VI. COMPENSATION
Your participation is voluntary and unpaid. SONA student will get 0.5 credits for their participation (please provide your email in this form)
VII. FREEDOM TO WITHDRAW
You are free to withdraw from this study at any time for any reason.
VIII. APPROVAL OF RESEARCH
This research has been approved, as required, by the Institutional Review Board for projects involving human subjects at Virginia Polytechnic Institute and State University, and by the Department of Computer Science.
IX. SUBJECT'S RESPONSIBILITIES AND PERMISSION
I voluntarily agree to participate in this study, and I know of no reason I cannot participate. I have read and understand the informed consent and conditions of this project. I have had all my questions answered. I hereby acknowledge the above and give my voluntary consent for participation in this project. If I participate, I may withdraw at any time without penalty. I agree to abide by the rules of this project
